Sending someone along as a Proxy to the AGM

I'm unable to attend & have someone attending on my behalf.

I'm a bit unsure as to what I need to fill out on the attendance card. (looks like the person attending signs the attendance card?)

On the proxy part, I assume i need to put the name of the person attending on behalf?

Do I then complete the resolutions & sign the proxy card, for the person attending to hand in?
